Data Missing

When I refilled my data in May I could only add a $15.00 package because I had too much left over now all of a sudden my data is down to zero. How ca this be when as you can see I knbow how to use it to keep my data in use fore a long time,DID youy guys erased my data when you sent me a early message about it would end on 6/9/2013? Something is wrong with this picture. I would like a history on where and when this data left my phone.
