Ford Freestyle 2005 Radiator Ventilator Not Turn On

When I am driving try high traffic my temperature gage start to go up and I stop on side and when I open hood coolant is coming from coolant container and radiator ventilator was off. Do I need to change coolant temperature sensor and tell me location of this sensor.
