Problem With Log In For Rewards Program

Rec'd ltr dtd 1/10/14 with instructions to log in @https://kp.kphealthworks.org.Both my husband and myself tried with this log in and were unable to get in (msg said "your search did not match any documents). We were aple to get in by logging on to kp.org, we found a box tat said log and both completed the questionsire. On 2/4/14 rec'd a call from Kaiser represenative (don't remember her name)she asked about the authorization page. She gave me the log in kp.kphealthworks.org and said we needed to put in our ID and password and on the top right of page we should see settings and click, tried that and did not get into any page that had those required things on it. I called the @ 2:28 PST and left a voice mail regarding our difficulty. We have received several more letters and have both completed this questionaire at least twice if not more. WE are at our wits end,we need to have this resolved as the deadline is approaching.This has been a nightmare and not the lest bit user friendly. PLEASE HELP. we need our $500.00. Yolonda L.
