Deadline For Unused Ticket

My husband and I were forced to cancel our trip on Air Canada because I was in an accident. I understand that I can extend the ticket for 90days beyond one year by paying $50. Does this mean that I must book a new trip by the end of this period, or does it mean that we must begin our travel by the end of the period?
