What Is The Phone Number, Address Or E-mail To File An Appeal?
Friend is in hospital and after surgery is ready to go to rehab. Rehab was denied, but they reviewed anoher patient's chart, not hers. She was told to file an appeal w Anthem. i am trying to help her move this process along.
