Can I Book 2 Seats For A 4x Sized Passenger

I know you can on Delta as well as American, as I've done it. Most agents understand the concept if you explain it to them. Book 1 seat under Last Name, First Name. Book 2nd seat as a 'companion seat' under Last Name EXST, First Name. You will not receive flight miles for the EXST reservation, but if you call Delta afterwards, they at times will provide you with 'bonus' miles into your account for the 2nd seat. They will not provide a refund for the cost of the 2nd seat if there are ultimately extra seats on the flight upon takeoff. I've gone all the way to the top with this question.
