Pet Traveling Alone

I'm trying to send my daughter a 4 month old toy poodle by himself. From Los Angeles CA (LAX) to Fort wayne Indiana airport. I need to no specifically what kind of traveling cage will he need ? What are the requirements needed in order to sent the 4 month old toy poodle by himself. Please and thank you !
