My Mother Has Been Declared Imcompetent!

When a parent has been declared imcompetent by the court system and she is in the nursing home, how can Embarq (Century Link) turn her phone on.  She will never be back to her home and I am her conservator.  Do I need to fax you a copy of the conservatorship given me by the court system or can THIS email do the trick.  Thanks Katherine H.
