To edit your date of birth:

Click the Edit Profile (Update Info) button at the top of your profile (timeline)
Enter your birthday in the Birthday field
Save your changes

You can submit a name change request through the "Name" section of your Account Settings page.

Please note that Facebook reviews each name change request and birthday change request, and may take some time before making a determination.
