Have Lost All Of My Profile Friends, Photos And Applications From My Page

have lost all of my friends, contacts, photos and application from my facebook profile. it appears that someone established another account using my information and is passing off as me. i need to correct and report this individual whomever they may be. i use facebook for personal and to connect with business associates.
