How do I reactivate a deactivated account if I don't remember the e mail address or user name or password

You need to request a password reset, by clicking the "Forgot your password?" link located near the "Login" button on the login page. Facebook will then send per your instructions a link to reset your password. (This information is from this Facebook page, which may provide additional guidance.)
If you've added a mobile number to your account, you can log in by entering that number in the Email field. Start with the country code and remove any leading zeroes and symbols.
You also may be able to get into your account by answering a security question or getting help from friends. Details are here.
