Repeated Hacking On My Fb Acct

I am repeatedly receiving TWO alerts to my cell phone that someone is signing on to my account at times when I'm not using fb. I've changed password 2-3x and this is still occuring. My cell had spyware on it, I've gotten rid of the phone, my old computer and also purchased a new router with new passwords on all. How is this repeatedly occuring. Just recently received TWO alerts (always same mess just comes thru back to back 2x every time??) that someone was signing onto my fb account with a new unknown device, it gives the city ( of my ex-fiance) along with a computer ip address. I downloaded itunes again and synched the new cell, could I have inadvertently downloaded the old spyware to my new cell? Does the apple store/itunes interact with fb somehow?
