How To Repair A Leaking, Sweating Toilet And Pipes?

I have a toilet that constantly runs and is leaking from the back of the toilet.  Plus, it is sweating.  Then, I found out that my pipes are sweating as well.  I want an inexpensive way to fix these problems.
