I Have A Rewards Account But I Dont Knot Have My Card How Do I Go About Getting

i am a rewards member go to kmart all the time but i need a new card how do i go about getting one. when i go to the store i just give them my phine #. pls i would like to get a rewards card. thank You    Carrie
