The PUK code(phone unlocking key) is derived from the meid number on the sticker under the battery so its unique to each phone. You have to get it from customer service. Call 1-800-550-2436 and dial 0 when the recording is finished.

This is simple :

1: ) first call your sims relevant customer care.

2: ) Second after that ask for puk code for your phone.

and chaos your problem solved .

if you operated gave a sim card then refer to the card that the sim was taken from it might be there or else you can your operator
