How Can I Terminate My Membership Now That I Lelft Chgo For Florida ?
T ried several times to cancel but have had no luck via internet. Eff 1/1/14 i wish tdo terminate my membership and be reimbursed for any montrhly charges made since. then. My membership # is 5046 at Oak Lawn, Ill facility.I had to lv the area and go to Florida where I currently reside My name Reggie Madon.>

- Go in to your local club front desk to tell them of your intent to cancel with a photo identification. It may take several days for the cancellation to process, but that should be all you need to do.
- If you no longer live locally, you will need to send your cancellation in by mail. Many clubs do not accept cancellation by facsimile, since they demand an original signature.
