'login Failed' Keeps Popping Up

For the last few days, whenever I unlock my phone a white box pops up that says "Login Failed" with my name underneath it, even though I'm not trying to log into anything. Then it automatically deducts 3mb from my memory. What's going on here and how do I fix it?
