You or your relatives can make the reservation online at the United Airlines website.

On the home page which will open in a new window, below the dates you will see Adults: and a numbers box.
Next to the box is Children.
When you click on Children it will link to the reservation page where you can select the age range for the child.
You can go ahead with your reservation if you'd like.
Be aware that if traveling internationally, your child will need other documentation.
Since this child is traveling with family, you don't need to worry about the Unaccompanied Minor Travel Policy.
Do be sure that the relatives have medical information and releases, just in case - not a policy of the airline.
