UA 427 on 3/4/12, PDX to ORD to YUL. I was forced to cancel this business trip because of severe flu. I have attached a scan of the letter from my doctor. The trip was booked by my company using a combination of HSBC Master Card miles and credit card payment. I am requesting a refund of the miles and credit card payment, or, a credit voucher for a future flight. Thank you for considering the my request. David K. Burtner.
