Creditable Coverage; Coverage On Secure Horizon Plan 3 In 2010.

I have a client who is getting charged $14/mo. for not having part D (drug plan)(crediable coverage penalty given) She can't remember what plan or carrier she had prior to 2010. 2010 she had Secure horizon Plan 3. QUESTION: Does or is this plan a MAPD? M/S (medical supplement) Can I access this plan coverage. Question 2: how can I find out what coverage and type during the 2006-2011 years?
