Hello,
this would depend on who your wireless provider is.
You can go to phoneclaim.com and choose your carrier
Once you choose the carrier, you will be given information on how to proceed with filing a claim for the phone
Please Note: If your device is lost or stolen you should still contact your carrier immediately to advise the device is no longer in your possession.
