Were Do You Send Rx Paper Claims To For Military Treatment Facility (mtf)?
I had called today 19 June 2012 to get the mailing address and recieved a different address compared to the memo we received dated 31 Oct 11. The memo has address as follows:
Express Scripts-Subrogation
P.O. Box 2863
Clinton, IA 52733-2863
When I called 800-962-7378, I was told Box 2872.

Express Scripts Subrogation forms for military treatment facilities can be sent to:
Express Scripts, Inc.
ATTN: Subrogation
P.O Box 2863
Clinton, IA 52733-2863
The other PO box mentioned (2872) is an Express Scripts box, but that is used for commercial clients and members. Military subrogation claims should use 2863.
