Workmans Comp Question

I have been diagnosed with carpal tunnel syndrome which is the result from working in the stockroom unloading boxes and processing clothes and shoes. The doctor gave me stints to wear at night to relieve the symptoms. Told me if they didn't work that surgery will be needed. Have been wearing them for 2 weeks and the pain is worse while I have them on. So looks like surgery will be done next. My question is do I discuss this with my manager or do I call human resources to discuss workmans comp? I feel bad about filing a claim but this injury is the result of my job. Also we don't have short term disability so I need to be compensated while unable to work to be able to still pay my bills. Hope someone gas the answer.
