Adding Frequent Flyer Miles From Recent Trip

I did not have my Continental Frequent Flyer number with me so the agent said to contact United when I got back home. How do I add the miles now? Also I have 2 Frequent Flyer numbers from Continental--can they be combined and if so how do I do that? Thank you
