Prior Reservation

I had a reservation from Medofrd, OR to DesMoines on or about last 8/14/12 and had to cancel due to documented medical reasons. Despite a surgeon's letter, I was told I could not be reimbursed, only use the flight w/i one year. I want a refund OR instructions how to utilize that pre-paid ticket. Please respond immediately. Thank you.
