Greeting - Your PUK code can be obtained by either taking out your battery and look for a 15 dgits number where you took out the battery (which you should write down - key it in after replacing the battery and turning the phone on) or by calling Net10 at 1-877-836-2368 . Please note that entering the PUK code 10 time incorrectly will block the sim card permanently (so please do guess it). Hopethis helps.
