Have You Billed My Insurance ?

Received invoice number 087251319 for Sheilia Lewis. Bill Code 1738 It shows you billed Medicare, but not Blue Cross/Blue Shield. They should pay the $12.45. You should have that info. If not it is R07737787 FEP
